The visual issues caused by an incorrect PD stem directly from classical optical physics. Under Prentice's Rule, any displacement of a lens's optical center generates an unintended prism:
Prism Diopters (Δ) = Distance from Optical Center (cm) × Lens Power (Diopters)
If a patient with a strong -6.00 diopter prescription has a PD error of 4 mm (0.4 cm), the resulting prismatic displacement equals 2.4 prism diopters. This is well outside clinical tolerance standards.
The immediate physical consequence is asthenopia: heavy, burning eye sensations and tension localized behind the eyebrows. Because the incoming light enters displaced, the brain struggles to fuse the disparate images transmitted by the left and right retinas. The vestibulo-ocular system falters, triggering motion sickness, spatial disorientation, and difficulty judging steps or curbs.
