On March 16, 2009, Richardson was taking a beginner ski lesson at the Mont Tremblant Resort, roughly 80 miles northwest of Montreal. During the lesson, she suffered a seemingly harmless tumble on a gentle slope. She wore no helmet and initially exhibited no signs of external injury. She laughed off medical assistance and returned to her hotel room.

Roughly an hour later, she complained of a severe headache and began showing signs of cognitive distress. An ambulance was dispatched, transferring her first to Centre Hospitalier Laurentien in Sainte-Agathe and then to Hôpital du Sacré-Cœur in Montreal. Neeson, who was filming Chloe in Toronto, rushed to her bedside. Doctors confirmed she had sustained an epidural hematoma, a fatal arterial bleed between the skull and the dura lining of the brain.

Richardson had fallen brain-dead. Neeson made the heartbreaking decision to fly her to Lenox Hill Hospital in New York City, allowing her mother, sister, and children to gather around her bed before life support was withdrawn on March 18, 2009. Neeson later spoke openly in an interview with Anderson Cooper about a mutual pact the couple had made: if either entered a persistent vegetative state, the other would ensure life support was discontinued. In honoring her wishes, the family donated three of her organs, her heart, kidneys, and liver, saving three lives.
